We know that forgoing dental appointments due to lack of dental insurance can result in more costly and time-consuming treatments in the future. That is why we created the North Oak Family Dental Membership Plan. For one annual fee you can be covered for exams, x- rays, and cleanings. It also gives a discount on all needed treatment.

Adult Preventative Plan
(Patients age 14 years and older with no history of periodontal disease)
- 2 Professional Hygiene Cleanings
- (Preventative Cleanings)
- 2 Oral Exams and Diagnosis
- 1 Emergency Exam
- 2 Oral Cancer Screenings
- 1 Fluoride Treatment
- Full Periodontal Screening
- Intraoral photos
- Any needed x-rays
- 1 Panoramic x-ray (every 5 years)
- 15% off any additional treatment needed

Adult Periodontal Maintenance Plan
(For patients with history of periodontal disease that has been treated and is stable)
- 4 Periodontal Maintenance Cleanings
- 2 oral exams with Diagnosis
- 1 Emergency Exam included
- 2 Oral Cancer Screenings
- 1 Fluoride treatment
- Intraoral photos
- Any needed x-rays
- 1 Panoramic x-ray (every 5 years)
- 15% off any additional treatment

Child Preventative Plan
(Age 13 and younger)
- 2 Professional Hygiene Cleanings
- 1 Emergency Exam
- 2 Oral Exams and Diagnosis
- 2 Fluoride Treatments
- Any needed x-rays
- Panoramic X-rays every 5 years (as needed)
- 15% off any additional treatment
$299/year
Terms and conditions
- This plan is a savings plan, NOT insurance, and cannot be combined with another dental plan, insurance or discounts.
- This plan is solely for your treatment with North Oak Family Dental Care. It cannot be used in any other office.
- In order to receive membership discount, full payment is due at time of service. Any service not paid in full will be billed at the customary rate.
- This plan is non-refundable. and non- transferable.
- Year membership starts from the date of complete annual payment and expires exactly one year after.
- Preventive and periodontal maintenance are different treatments and must be diagnosed by Dr. Bland to decide which plan is needed to meet patient’s dental needs.
- North Oak Family Dental Care has the right to change or alter the membership plan prior to patient’s renewal and patient will be informed of changes prior to renewal.
